Sometimes students have extenuating circumstances that compel them to miss school. These circumstances are further defined below. In these cases, students may apply to have their absence waived. An attendance committee hears these requests and makes a determination on what days will be waived.

To have absences considered for waiver by the IC Imagine Upper School Attendance Committee, you and your parent/guardian must complete the following information for all ABSENCES that exceed 7 days. Excused absences are not automatically waived.

Complete the form below and attach documentation for absences. Forms should be turned in as soon as possible after the 7th absence.
Make sure your parent/guardian has provided information for a Release of Information if this is a request due to health issues. Your parent and physician will have to sign the Release of Information Form that is sent via Adobe Sign in addition to filling out this form before a waiver for health issues will be considered.
Check the category of your absence(s) and provide complete date and absence information.
Incomplete attendance waivers and attendance waivers without documentation will not be considered.

Definitions of Reasons (must choose one)

Acute Illness: An illness of 4 or more days due to acute illnesses such as influenza, etc. Other examples include emergency illnesses requiring hospitalization and/or surgery. Doctor’s notes must be provided for all days missed.

Death in Immediate Family: Up to 3 days may be waived. Obituary/service program must be provided.

Court appearances: Only waived if a student is subpoenaed as a witness in a court case. Court appearances for involvement in criminal activity will not be waived.

Chronic illness: Requires a doctor’s note that includes explanation of the illness, specific days missed from school and the duration of the chronic illness. Parent notes may be accepted if there is a clear doctor’s note stating that the student may not be seen every time an episode occurs, i.e. debilitating migraines, asthma attacks, etc.
You must submit documentation for the waiver even if you submitted documentation to the attendance email to excuse your absences. **Students with chronic health issues must provide documentation to the school nurse at the BEGINNING OF EACH YEAR.

Teacher Supported Request: Up to 3 days may be waived. If a student has completed all work in a course and has a grade of C or better, a student may request that a teacher fill out an attestation form. The student must talk with the teacher and ask them to fill out a support attestation in addition to this form. The teacher will provide the attestation directly to school administration. If no attestation is provided by the teacher, the teacher supported waiver will not be approved.

Other: Compelling circumstances may be considered on a case by case basis by the attendance committee. Parents/guardians should provide all available documentation.

There is a difference between an excused absence and a waived absence. An excused absence means that the absence will not count against the student/parent when considering the NC Compulsory Attendance Law; however, it still counts towards the 7 day limit that students have before they MUST make up the time. A waived day does NOT count towards that limit.
